Assistance
for Seniors, Inc. provides care management for Seniors and the Disabled. Our
professional staff works with clients, family members and other interested
parties to assess individual needs and provide, or arrange for, needed services
to help our clients to maintain their independence. We provide in-home assessment
of client needs, then identify coordinate and monitor services to assist
the client with remaining in their home.
Assistance for Seniors
goals are to:

Maintain the client's quality of life.

Assist the client to remain safe and independent in the home.

Assist family members, including those who live outside of the area,
with the care of the elderly and dependent.
Services Provided:

Comprehensive assessments of the individual's health and safety needs
by a professional geriatric care manager.

Review of living arrangements to maintain independence.

Development of a
care plan
that involves the family and the client, and
uses community resources to provide a cost effective support system.

Consultation and referral to
appropriate
professional services.

Coordination of support services such as: companion or home health aide
services, senior day care, transportation, meals, shopping, obtaining
prescription medications, home maintenance, assistance with bill paying and
completing insurance forms.

Facilitation of distant
care giving
to enable family members who live
outside the area to stay actively involved in the care of their family
members.

Advice on appropriate and cost effective living arrangements.

Identification
of services to permit an individual to safely return
home after a stay in a rehabilitative facility.

Ongoing monitoring of a client in the home.
Assistance for Seniors also provides information and guidance for long
term placements if that is required. Our professional staff will accompany
caregivers during visits to nursing homes or alternative living sites to
help in the selection of the most appropriate facility. We can provide ongoing
monitoring of the quality of care at the selected facility.
